* fix(runtime): prevent dual execution after forced cancel

- keep bookkeeping until task termination
- block restart while any execution task is still alive
- make execution registration atomic under lock
- avoid premature cleanup on cancel timeout
- add regression tests for forced-cancel restart scenarios

* chore: ruff format and import order
